Sumangguni sa CIO staff, humingi ng Barangay ID Form at sagutan ang mga hinihinging impormasyon.

Kukuhanin ang sinagutang form, susuriin kung tama ang mga nakatala at tatatakan ng kaukulang petsa at oras kung kailan isinumite ang form.

Kung may dalang 1x1 o 2x2 picture, ipadikit sa isusumiteng form ngunit kung wala, magpakuha ng picture sa CIO staff.

Kukuhanin ang picture na dala ng kliyente at kung wala ay kukuhanan ng picture ang mga humihingi ng ID.

Makipag-ugnayan sa CIO staff upang alamin kung kailan maaaring makuha ang ID.

Magtatakda ng kaukulang araw kung kailan maaaring balikan ang ID.

Makipag-ugnayan sa City Information Officer para mapagkasunduan ang nilalaman ng hinihinging mensahe/talumpati.

Magtatakda ang City Information Officer ng araw kung kailan babalikan ang hinihingi.

TRICYCLE FRANCHISING AND REGULATORY BOARD

              By virtue of City Ordinance No. 07-9 “Tricycle Franchising and Regulatory Board or TFRB was created composed of one chairman and two (2) members coming from the members of Sangguniang Panlungsod. The Vice Mayor acts as the chairman with the Chairman of Committee on Public Utilities and Facilities and Chairman of the Committee on Peace and Order as its members. The office which is headed by the Vice Mayor has 9 staff members.

  1. I. Duties:
  2. A. TFRB has the power to accept process, evaluate, approve or disapprove franchise applications and issue and sign franchises and other duties that maybe assigned by the Sangguniang Panlungsod.

              The tricycle shall be valid from the date of its issuance up to such date indicated in the franchise.

              In no case shall the franchise be effective for more than 3 years.

              A franchise maybe revoked or cancelled anytime for any violation of the provisions provided in the ordinance or of any existing law.

  1. B. TFRB is also mandated to grant Special Permits to Motorized Tricycles for Hire coming in the jurisdiction of Tanauan from other municipalities and nearby towns and Motorized Tricycles exclusively used as school service. Special Permit issued is valid for one year renewable every January. P350 must be paid to the Treasury Department when applying for Special Permit.
  2. II. Programs Initiated:
  3. A. Positive reinforcement program

- A program intended to give due recognition to drivers / operators who returned lost item to the office. This serves as commendation for the act of honesty which they manifested.

  1. B. Monthly raffle draw

-To motivate all the tricycle drivers/operators to renew their franchise on time, monthly raffle draw is held every month.

  1. C. “TFRB / TODA PARA SA PAGKAKAISA” Program

- A program which aims to foster a better office-client relationship. It intends to establish close coordination with the members of TODA Federation which is our partner for growth and progress in the city.

III. Our office is open from 8:00 AM to 5:00 PM daily. NO NOON BREAK

  6. Current Barangay Clearance
  7. Current Police Clearance
  8. 2 pcs. 2x2 colored picture
  9. Professional Driver’s License

  1. Affidavit of change motor
  2. Toda Certification
  3. Federation Clearance
  4. OR/CR or Sales Certification

For Change Operator

  1. Original franchise

  1. Valid Franchise (from the point of origin)
